Before Your Appointment
Our office requests you send the following information in advance of your appointment. If you are unable to send these items prior to your appointment, please bring them with you.
A disc with your MRI studies
Clinical notes from your referring doctor
Any recent test results (blood tests, spinal fluid, evoked potentials)
A list of medications, including doses, start date, ordering physician and reason for taking the medication
We recommend that you contact your insurance company to verify your benefits and find out if a referral from your primary care doctor is required for your visit. It is important that you bring your health insurance card to your appointment.
To save time you can download and complete the intake forms and bring these with you to your appointment. If you are unable to complete them before the appointment, please allow ample time to complete them upon arrival.
What to Expect When You Arrive
When you arrive for your appointment, you will check in with an administrative assistant. If you have not completed the intake forms prior to your arrival, we will give you the forms to be completed. These forms should be updated at every visit. Please notify the office of insurance changes immediately.
Before your appointment with Dr. Katz, Dr. Bouley or Dr. Lathi, you will see the Clinical Assistant. She will check your vital signs, assess your BMI, review your list of medications, and perform basic multiple sclerosis testing. This testing includes a peg test for manual dexterity, a short and long walking test, a SDMT, and a vision test. When this has been completed, you will see your physician. Following your appointment, you may meet with one of our clinical coordinators or nurses to review any medication changes and implement the plan made with your doctor.
